• A0426 - Advanced Life Support 1: $700
• A0427 - Advanced Life Support 1, Emergency: $800
• A0433 - Advanced Life Support 2: $900
• A0434 - Specialty Care Transport: $1,000
• Refusal, Against Medical Advice (AMA - medical assessment performed, vitals obtained, Medication administered on scene): $125
• Consumables - Cost of consumable used

This ambulance provider fee schedule is posted in pursuance of 202 KAR 7:575. This fee schedule is subject to change by King's Daughters Medical Transport. Any change to this fee schedule will be updated within 15 days of the decision by KDMT.
