Several of our primary care locations recently received Patient-Centered Medical Home recognition from The National Committee for Quality Assurance. These locations join our first two recognized practices, Grayson Family Care Center and Portsmouth Family Care Center:

The Patient-Centered Medical Home (PCMH) is a model of care that puts patients at the forefront of care. PCMHs build better relationships between people and their clinical care teams.
Patients have better outcomes when all members of the care team share information and collaborate with each other – and patients. Research shows that patient-centered medical homes improve quality, reduce costs, improve the patient experience, and improve staff satisfaction.
Facts from the National Committee for Quality Assurance include:
- Fragmented care results in poorer care. Higher rates of fragmentation are associated with higher costs, lower quality and higher rates of preventable hospitalizations.
- Integrated care produces better outcomes. Communicating information for shared patient populations results in better care.
- Payers are increasingly supporting PCMH. Because the PCMH model can help patients avoid costly complications, public and commercial payers are increasingly turning to the PCMH model of care.
